Membership Form

I the undersigned, do hereby warrant that I have authority to sign for all employees and agents of the above identified company and or person (sole trader) having the above identified address. I hereby enter into this agreement on behalf of all employees and agents of this organisation, including on behalf of myself (jointly “Recipient”). Good and valuable consideration has been received in the form of an opportunity to learn Wu Xing Dao.

The Recipient will not, without prior written approval of the party disclosing information, Grant Mundy (Owner), or an authorised representative thereof, disclose or in any way make known, reveal, report, publish or transfer to any person, firm, corporation or utilise for competitive or any other purposes any information, documentation or know-how relative to the disclosed information designated as secret; inclusive of the program, plans, methods, specifications, techniques, articles, patent applications and other materials relating to the machinery, equipment, processes and products invented or used by the Owner. This agreement will expire the latter of fifty (50) years, or when the information becomes public through no fault of the Recipient.

I intend to be legally bound and do hereby agree to be legally bound for myself and for all successors in interest I may have, by this Contract, Waiver and Release of Liability, and hereby agree to hold harmless and indemnify, Inner Energy Fitness and Wu Xing Dao Kung Fu, its instructors, as well as any coordinators, whatsoever acting on or in behalf of the aforementioned entities and persons against any claims for damages or other claims for injuries or losses of any kind suffered by me or any others directly or indirectly arising out of my practice, instructions, or any other activity related to this program, as well as participation in this program or travelling to or from this program or any other activities related to this program.
